Health is the product of many factors, only some of which are driven by healthcare. Yet we continue to pay more every year for medical care while consistently under investing in the human and social services with the greatest potential to prevent illness and improve individual and population health. Absent greater attention and resourcing for upstream solutions that address health-related social needs, we individually and collectively suffer the consequences.

Our Health and Human Services group creates the conditions for healthcare organizations and community-based service providers to build robust business partnerships that deliver sustainable solutions – improving health outcomes, addressing health disparities, and lowering costs at scale.


Solutions

Community-Based Organization Sustainability Solutions

We work with community-based service organizations (CBOs) to access new sources of revenue – and sometimes financing – that allows them to sustainably operate and grow programs that address health-related social needs and deliver whole-person care.

Managed Care Organization Market Growth

We support managed care plans/organizations (MCOs), especially with Medicaid and duals products, by helping them access new markets and advance innovative, member-centric, high-impact opportunities in markets they already serve. 

 

SDOH Strategy and Financing

We help partners to prioritize investment in and development of a compelling and scalable solution (and a best-fit provider partner) that demonstrates a commitment to addressing social determinants of health (SDOH), program / payment innovation, and supports strategic goals.

State Medicaid Innovation

In states adopting Medicaid payment policies or other mandated investment in SDOH, we work with implementation stakeholders across sectors to create sustainable business partnerships that support successful execution and optimize the outcomes of these new policies.

Housing and Homelessness

Develop upstream solutions to divert evictions and other drivers of housing loss and expand capacity for evidence-based care models like medical respite

Mental Health and Substance Use

Develop new earned revenue opportunities for programs that integrate physical, mental, and behavioral health for specific populations (e.g., pregnant / parenting women with substance use disorder).

Healthy Aging

Craft novel solutions to provide peace of mind and meaningful support services to aging individuals with a deep respect for personal aging choices and a focus on engaging family / community support for whole-person health

 

Food Security

Design programs, develop operating models, and advance partnerships to create durable solutions to food insecurity and access to fresh, local food that is critical to health.

Environment and Health

Broaden the evidence base and application of how nature and natural infrastructure can deliver health benefits and community resilience, in recognition that our health is linked to the health of the environments in which we live.

Community Well Being and Economic Resilience

Provide sustainable revenue structures for workforce development programs and expand access to job placement services, career coaching, and financial services to address barriers to self-sufficiency.

The amount of need within communities we serve has been increasing year over year and became especially acute during the pandemic. We stretch every one of our dollars to meet the needs of the community and pay our staff a fair wage for the demanding, important work they do. One of the only ways to enhance services that meet increased community need is through value-based arrangements and the shared understanding that there is tremendous opportunity within the health system to provide for better outcomes and better cost solutions. I am extremely proud of our ability to create a value-based arrangement to significantly expand medical respite capacity in Washington, DC.
