While important to tailor social determinant of health (SDOH) solutions to local conditions, many parts of the country experience similar needs (e.g., housing insecurity, poor access to behavioral health and substance use treatment). This presents an opportunity to build network capacity and enterprise infrastructure in a way that achieves greater consistency and alignment in service models across multiple markets.

Our SDOH strategy and financing solution guides managed care plans, and other organizations committed to addressing community care and wellness gaps, toward high-impact program and sustainable payment models that enable scale.

We provide a focused analysis of potential solutions, including comparative cost and impact assessments. We also assess market conditions, review key drivers effecting program success, and identify risk mitigation strategies. Finally, we provide a customized financial model and strategic financing plan for capitalization.

Financing Strategies for Community Health Workers

QV is working with the Centers for Disease Control and the Institute for Public Health Innovation to ensure that community health workers (CHWs) are deployed to the communities and populations in rural Southwest Virginia that would benefit most from their support. We are also developing financing strategies to support the long-term sustainability of CHW programs in the region.

Addressing Cancer Treatment and Mortality Disparities Among Races in Washington, DC

Black women have a 12% higher cancer mortality rate than other racial groups. To help address cancer treatment and mortality rate disparities, QV is working with the Jane Bancroft Robinson Foundation, Black Women Thriving East of the River, and community members to design, develop, and deploy a financing strategy that delivers new navigation support to Black women in Washington, DC.

Client Testimonials

Neighborhood Service Organization is thrilled to have worked alongside Quantified Ventures to develop a novel value-based financing structure to launch a best-in-class medical respite program serving unhoused people in Southeast Michigan. We combined NSO’s strong track record in social services and deep knowledge of Detroit with QV’s expertise in building healthcare partnerships that catalyze impact to design a solution that meets a critical need in our community. QV’s approach ensured that our initial value-based contract with Molina Healthcare of Michigan is replicable, which streamlines contracting with other Medicaid Managed Care Organizations in Michigan to grow the NSO medical respite program. We’re proud to see Detroit leading the way on the medical respite model and we believe this solution is one that is scalable and can bring value to communities of all sizes throughout the U.S.
— LINDA LITTLE, PRESIDENT & CEO, NEIGHBORHOOD SERVICE ORGANIZATION
The amount of need within communities we serve has been increasing year over year and became especially acute during the pandemic. We stretch every one of our dollars to meet the needs of the community and pay our staff a fair wage for the demanding, important work they do. One of the only ways to enhance services that meet increased community need is through value-based arrangements and the shared understanding that there is tremendous opportunity within the health system to provide for better outcomes and better cost solutions. But first we have to think differently about the system and the role of community-based organizations in the system. This only gets done with strong partners like Quantified Ventures, Humana, and Volunteers of America bringing their best skills and attributes to the table and working with one another to achieve the system transformation we seek. One of the things I appreciate most about our partnership with Quantified Ventures is our shared mission – at the end of the day we are focused on providing services to support healthy individuals and healthy communities.
